Around 87% of younger senior citizens state that they are satisfied or very satisfied with their implants. However, despite impressive success rates, an implant is still lost every four minutes, which translates into 140,000 cases per year, according to experts (Wagner 2010). Undetected inflammation and tissue breakdown processes are among the most frequent causes of implant loss. On average, peri-implant mucositis is detected in around 47% of implant patients and almost 30% of implants (Lee et al. 2017). A recent meta-analysis indicated that peri-implantitis was found in almost 20% of implant patients and 9% of implants (Lee et al. 2017).

Some of the evidence-based conclusions drawn by the authors of the new peri-implantitis classification (Schwarz et al 2018):

    • Peri-implantitis is a pathological condition that occurs in the tissue around implants. It is characterized by inflammation of the peri-implant soft tissue and progressive bone degradation.
    • The initial stages of peri-implantitis can occur at an early stage during aftercare following implant treatment.
    • Peri-implantitis may progress at a faster rate than periodontitis and is usually more difficult to treat.
    • There are strong indications that patients with chronic periodontitis, poor plaque control and a lack of regular oral hygiene are at an increased risk of developing peri-implantitis after their implants are placed.

The Best Protection Against Peri-implantitis

The best protection against peri-implantitis is early detection of collagen tissue breakdown around the implant. The earlier a hidden inflammation can be detected, the easier it will be to treat. What’s more, the earlier the affected areas of the implant and periodontium are specifically and professionally treated, the higher the chances that the patient will make a full recovery. Therefore, every opportunity for early detection and prophylaxis should be utilized to ensure that complications or implant loss can be combated through treatment at an early stage. However, it is not possible to distinguish healthy from unhealthy peri-implant tissue using probing or pocket-depth measurement alone.
